Reactivate your employment status.
To be eligible to be reactivated with Elite Health Solutions the following must be true:
- Employee must have worked within the last 2 years.
- Employee must be in good standing with Elite
- Employee must be able to turn in current credentials
If it has been more than two years since you have worked with Elite, the application process must be started again. Click here to update your application. Once your application is updated, an interview can be set.
To get back on Elite Health’s roster, please fill out your name and contact information below and a representative from Elite will be in contact with you very shortly. Please note, Elite has the right to deny any reactivation request for any reason.
